Along with your registration form, we please ask that all new patients attach some form of proof of address (this can be a recent utility bill, tenancy agreement, council tax bill etc) and proof of identity (birth certificate, driving licence or passport)​. If you are unable to provide this documentation it will NOT prevent you from registering. However, please state in your email why you have not attached the required documentation.

From April 1st 2015 all patients have to have a named 'Accountable GP'. For administrative purposes only, patients are allocated a GP. However patients can continue to see the GP of their choice and we can amend their records to reflect this. We do value continuity and advise patients to remain with the same doctor when possible.

We accept all new registrations from any patient living within our catchment area. Patient post code is the only criteria and we do not discriminate on grounds of race, gender, social class, age, religion, sexual orientation or medical fitness.  We ask for proof of address; this can be a recent utility bill, tenancy agreement, council tax bill etc and proof of identity ie birth certificate, driving licence or passport but please note that the absence of these will NOT prevent you from being able to access medical care at the surgery.

You will be asked to fill out a registration form and a health profile form. We invite people with complex problems to come for a registration health check.

 Please check that you are within our catchment area (map shown below) before registering.
