New Patient Registration

REGISTER ONLINE

We accept new registrations from any patient living within our catchment area. We do not discriminate on grounds of race, gender, social class, age, religion, sexual orientation or disability.

You will not be asked for proof of address, immigration status or identity. Proof of identity is needed if you want to have online access to your records.

You will be asked to fill out a registration form and a health profile form. We invite people with complex problems to come for a registration health check.

From April 1st 2015 all patients are required to have a named 'Accountable GP'. For administrative purposes only, patients are allocated a GP. However, patients can continue to see the GP of their choice and we can amend their records to reflect this. We do value continuity and advise patients to remain with the same doctor when possible.
