Patient Registration

writingPatients are now registered with the practice rather than a specific doctor. We do, however, still value continuity and advise patients to remain with the same doctor when possible.

We accept all new registrations from any patient living within our catchment area. Patient post code is the only criteria and we do not discriminate on grounds of race, gender, social class, age, religion, sexual orientation or medical fitness.  We ask for proof of address; this can be a recent utility bill, tenancy agreement, council tax bill etc and proof of identity ie birth certificate, driving licence or passport.

You will be asked to fill out a registration form (or you can simply bring your NHS card) and a health profile form. We invite people with complex problems to come for a registration health check.
